Privacy of Your Protected Health Information
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.

Our Legal Duty
We are required by federal and state laws to maintain the privacy of your protected health information (PHI). We must also provide this notice describing our legal duties and privacy practices.
We follow the privacy practices described here while this notice is in effect. This notice takes effect April 14, 2003, and remains in effect until replaced. We reserve the right to change our practices and make the new terms effective for all PHI we maintain.

Uses and Disclosures of Protected Health Information
We may use and disclose your PHI for treatment, payment, and health care operations as allowed by HIPAA.
Examples include coordination of care between providers, billing your insurance, and operational activities such as staff training and quality improvement.
We may also contact you about appointments or other health-related information using the communication methods you have approved.
Appointment Reminders by Text (SMS):
With your consent, Ansari Pediatrics may send appointment reminders and updates via text message (SMS), phone call, or email. These communications are for informational purposes only. Message and data rates may apply. You may opt out at any time by replying STOP to a text message or by contacting our office directly.
We will share PHI with trusted “business associates” (such as billing or lab services) who must safeguard your information through written agreements.
Other uses and disclosures will only occur with your written authorization, unless required or permitted by law (for example, public health reporting or law enforcement requests).
Your Rights
You have the right to:

SMS/Text Messaging Terms

By opting in, you consent to receive appointment reminders and updates by text message (SMS) from Ansari Pediatrics at the phone number you provide.

Message Frequency: Typically 1–3 messages per appointment (confirmation, reminder, or reschedule notice).

Message & Data Rates: Standard carrier rates may apply.

Opt-Out: Reply STOP at any time to end text messages. You may also call or email the office to change your communication preferences.

Purpose: Informational reminders and updates only — we do not send marketing or promotional texts.

Privacy: Your contact information is stored securely and used only for office communications as described in this notice.

Carriers: Wireless carriers are not liable for delayed or undelivered messages.

Questions and Complaints
If you have concerns or believe your privacy rights have been violated, you may contact us directly. You also have the right to file a complaint with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
