What to Bring to Your First Visit
We work hard to ensure your child receives prompt medical care in a comfortable, child-friendly office setting. To help expedite your first appointment, please bring:

Immunization records and be prepared to review your child and family’s medical history, as well as insurance information.

Do not use the link above for your kids. Please simply click on This Link or provide your email address to our front desk staff during your next visit. We will send a secure, direct “Parent-Proxy” invitation link straight to your smartphone to link all your children into one single portal view!
What to Expect
On the day of your child’s first visit to our office, we recommend arriving a few minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office. You may also be asked to complete or confirm your Appointment Reminder Consent Form during your first visit. This allows us to send you convenient text message (SMS) reminders for future appointments.
